Discharge Planning Committee Meeting

Short Description: Addresses discharges of people who are homeless from hospitals, incarceration or any institution

Discharge Planning works with:

  • Local service agencies and government to create policies designed to prevent individuals from being discharged into homelessness from medical/mental health facilities, foster care, and jails or prisons;
  • Kern’s Community Corrections Partnership to help prevent discharge into homelessness among non-violent offenders released from overcrowded state prisons back to the community under county supervisions (per California Assembly Bill 109, aka California Safety Realignment Act);
  • Kern’s Methamphetamine Reduction Task Force to ensure increased access to meth recovery for services for homeless addicts; and
  • Other agencies to create policies designed to prevent individuals from being discharged into homelessness; identifies opportunities to improve outcomes, maximize available resources, and decrease duplication of services among service providers.
